According to a message to its members issued today by the American Urological Association (AUA), the AUA and the American Society of Clinical Oncology (ASCO) are archiving their collaborative 2008 guideline entitled Use of 5-alpha Reductase Inhibitors for Prostate Cancer Chemoprevention.
This guideline has now been removed from the ?Clinical Guidance? section of the AUA?s website, but is still available in the ?Archived Guidelines? section. According to the message sent to the AUA members, ?This decision is in response to the U.S. Food and Drug Administration?s denial of the supplemental New Drug Application for dutasteride for prostate cancer chemoprevention? ? a denial issued by the FDA in late January 2011.
The AUA further recommends to its members that using 5-alpha reductase inhibitors (5-ARIs) like dutasteride and finasteride for prostate cancer prevention ?is not FDA-approved and should be performed with caution.? The message concludes by noting that 5-ARIs ?maintain an FDA-approved indication to treat symptomatic benign prostatic hyperplasia.?
The ?New? Prostate Cancer InfoLink believes that some urologists and urologist oncologists will continue to prescribe 5-ARIs for the prevention of clinically significant prostate cancer in carefully selected individuals who are at potentially high risk for prostate cancer, despite the fact that the FDA denied a formal approval of dutasteride for use in this indication. The ?archiving? of the joint ASCO/AUA guidance document is probably (as much as anything else) a way for the two organizations to protect themselves legally from possible future accusations that they continue to ?recommend? the use of 5-ARIs for chemoprevention of prostate cancer. We do, after all, live in a highly litigious society.
The evidence regarding the actual effect of 5-ARIs in the prevention of prostate cancer and the risk that this use of ARIs may actually increase risk for high-grade prostate cancer is still debatable and controversial, with strong opinions on either side of the issue.

XBMC 12 beta 2 now available with official Android support
The folks at XBMC have released the second beta for version 12 (Frodo) and we finally see official Android support. XBMC for Android has been "unofficially" available on and off, as the open-source project code is easily compiled and redistributed. Official support means the right people are working on it, with organized version control and bug tracking.
In case you're not sure what XBMC is, think of it as the ultimate cross-platform digital media hub. Support for most fie types on most operating systems, compatibility with a multitude of remotes, and easy network streaming make XBMC a pretty big deal. Getting official support for Android is a pretty big milestone for the platform as a whole.
Remember, it's still in beta, and you're forewarned that the first load is ultra-slow. Also, right now XBMC doesn't work with Android 4.2, but we're told beta 3 corrects this and is "right around the corner". For more information and the download, hit the links below.

VATICAN CITY (AP) ? Benedict XVI, the pope known for his hefty volumes of theology, is now trying brevity ? spreading the faith through his own Twitter account.
The pontiff will tweet in eight languages starting Dec. 12 using his personal handle (at)Pontifex, responding live to questions about faith during his weekly general audience, the Vatican said Monday.
Questions can be submitted to (hash)askpontifex, and the pope will likely respond to three to five of those sent from around the world, the Vatican's communications adviser Greg Burke told a news conference.
The pope sent his first tweet last year from a Vatican account to launch the Holy See's news information portal, part of efforts to increase the church presence in social media and spread the faith. A personal Twitter account for the 85-year-old Benedict has been the subject of speculation ever since; Monday's news conference was packed in an indication of the interest it has generated.
Within two hours of the announcement of the pope's handle, he had already garnered nearly 60,000 followers.
Burke said the handle (at)Pontifex was chosen because it not only means pope in Latin, but also bridge-builder, suggesting unity. How often will the pope tweet? "As often as he wants," Burke said, adding that he hoped the tweeting would be frequent and regular.
While the pope will push the button himself on Dec. 12, subsequent tweets will be sent by someone in the Vatican's secretariat of state. They will, however, all be approved by the pope, officials said.
"It's always going to have his engagement and his approval," said Monsignor Paul Tighe, the No. 2 in the Vatican's social communications office. "Not physically, but from his mind."
Currently a host of Twitter accounts use the pope's name, purporting to be his personal account. The (at)Pontifex account, however, is certified as the only official papal Twitter feed, Tighe said. No special security arrangements have been taken to prevent the account from being hacked; Burke said Twitter has many celebrity clients.
"They seem to be able to take care of them. We assume they'll be able to take care of us as well," he said.
Papal tweets will be sent simultaneously in English, French, Spanish, Portuguese, Italian, German, Polish and Arabic, Burke said. Monsignor Claudio Maria Celli, the president of the Vatican's social communications office, said he hoped to add Chinese.
He stressed that the papal tweets aren't to be considered infallible teachings, merely "pearls of wisdom" in the pope's own words.
The Vatican has been increasing its presence in social media, using YouTube channels and Facebook pages for special events and Twitter to engage believers and nonbelievers alike, particularly the young.
The Vatican decided against using a personal Facebook page for the pope because they thought it was too personal an interaction and would require more manpower to keep updated.

Geithner predicts Republicans will accept higher tax rates
WASHINGTON (Reuters) - Treasury Secretary Timothy Geithner pressed Republicans to offer a plan to increase revenues and cut government spending, and predicted they would agree to raise tax rates on the wealthiest to secure a deal by year-end to avoid the "fiscal cliff."
In a blitz of appearances on five Sunday morning talk shows, Geithner insisted that tax rates on the richest needed to go up in order to reach a deal, a step Republicans have so far resisted, and he dismissed much of the contentious rhetoric from last week as "political theater."
"The only thing standing in the way of would be a refusal by Republicans to accept that rates are going to have to go up on the wealthiest Americans. And I don't really see them doing that," Geithner, who is leading the Obama administration's fiscal cliff negotiations, said on NBC's "Meet the Press."
The comments mark the latest round of high-stakes gamesmanship focusing on whether to extend the temporary tax cuts that originated under former President George W. Bush beyond their December 31 expiration date for all taxpayers, as Republicans want, or just for those with incomes under $250,000, as President Barack Obama and his fellow Democrats want.
Republicans, who control the House of Representatives but are the minority in the Senate, have expressed a willingness to raise revenues by taking steps such a limiting tax deductions, but they have largely held the line on increasing rates.
A handful of House Republicans expressed flexibility beyond that of their party leaders about considering an increase in tax rates for the wealthiest, as long as they are accompanied by significant spending cuts.
But most House Republicans refuse to back higher rates, preferring to raise revenue through tax reform.
"There's not going to be an agreement without rates heading up," Geithner said bluntly on CNN's "State of the Union."
The scheduled expiration of the Bush-era tax cuts and automatic reductions government spending set to take hold early next year would suck about $600 billion out of the economy and could spark a recession. The Obama administration and Congress are engaged in talks to avoid the fiscal cliff with a less-drastic plan to reduce U.S. budget deficits.
WHO SHOULD PAY?
Geithner's Sunday interviews are part of a broader push to build public support for the Democrats' position in the negotiations. Obama has made campaign-style appearances, including visiting a Pennsylvania toy factory on Friday where he portrayed Republicans as scrooges at Christmas time.
While breaking no new ground on the Obama administration's position on Sunday, Geithner repeatedly urged Republicans to provide their own plan.
"They said they're prepared to raise revenues but haven't said how, or how much, or who should pay," Geithner said on NBC.
In an interview with the Wall Street Journal on Friday, the Republican leader in the Senate, Mitch McConnell of Kentucky, asked Democrats to accept an increase in the Medicare eligibility age, impose higher Medicare premiums for the wealthy, and slow cost-of-living increases for Social Security.
At least one of those suggestions appears to have White House support. On CNN, Geithner said the administration's proposal included a modest rise in premiums for higher-income Medicare beneficiaries.
"What we can't do is sit here trying to figure out what works for them," Geithner said. "The ball really is with them now."
The administration has said it is willing to find savings in the Medicare and Medicaid healthcare programs for the elderly and poor, but Geithner reiterated in an interview with ABC's "This Week" that it would only be open to looking at changes in the Social Security retirement program outside of the context of a fiscal cliff deal.
